Hospital Discharge Checklist
Questions families can ask before leaving the hospital and as they prepare for care at home.
Review medications
Confirm the current medication list, dose, schedule, new or stopped medications, and who to contact with questions.
Understand monitoring instructions
Ask what measurements or symptoms to watch, how often to record them, and what changes warrant a call or urgent care.
Confirm follow-up
Write down which providers need to be seen, when appointments should happen, and whether labs or imaging are needed.
Plan nutrition, contacts, and responsibilities
Clarify any food or fluid instructions, save important contact numbers, and decide who will coordinate tasks and appointments.
